We regret to inform you that Choice Family Urgent & Primary Care will be closing its doors indefinitely on Monday, August 10, 2026. Appointments and walk-ins will be available through Friday, August 7, 2026, until 5:30 p.m.
If you are currently an active member, your membership for billing purposes terminated on August 3, 2026. You may continue to access services through Friday, August 7, at no cost.
It has been our pleasure to serve your healthcare needs these past three years, and we wish you good health in the future.
